Cutis "Diagnose This" Question Features Medical Student's Case Report

Congratulations to UTMB medical student Michael Ryan (MS3) on a prior case report of his being turned into a "Diagnose This" challenge question in the most recent edition of Cutis. (September 2018, page 184). Michael had originally published the case report in Cutis earlier this year (2018 May;101(5):362, 365-366) in order to highlight a rare presentation of a common disease.

Can you figure out the diagnosis for this case?  You can find the question and answer online at http://bit.ly/2KOAeu0 A link to the case report will also show up after an answer is submitted.
